Quadrant—

ICER is undefined when ΔE = 0, and is meaningless on its own in the NW and SE quadrants. Both give a negative ratio, so dominance decides, not the number. The mirror trap: NE and SW both give positive ratios, and the same ICER value means opposite things. In the SW you are being asked to sell health for savings, so the decision rule reverses. Net monetary benefit (NMB = λ·ΔE − ΔC) has no such ambiguity, which is why it is the cleaner formulation. Scenarios are illustrative teaching figures, not extracted results. Check the papers.
